RAMQ: How Many Days You Can Leave Quebec
Loading article…
Quebec residents can leave the province for extended periods without losing public health coverage, but only up to a point. Cross the RAMQ 183-day threshold in a calendar year and you lose your eligibility for that entire year, not just for the days you were away. This guide explains how the Régie de l’assurance maladie du Québec counts your absences, which ones do not count at all, and what the plan actually reimburses when you receive care elsewhere.
The 183-Day Rule Explained
To keep your RAMQ coverage, you must not be absent from Quebec for 183 days or more in a calendar year, according to the Régie de l’assurance maladie du Québec. The reference period runs from January 1 to December 31, and it resets every year. Your absences do not need to be consecutive: several separate trips add up toward the same total.
If you pass that threshold, you lose the status of a person who resides in Quebec. Consequently, you lose eligibility for the whole year of the absence, and you must reimburse the covered services you received during that year. The penalty is retroactive, which is what makes this rule harsher than most travellers expect.
Which Days Actually Count
The tally is narrower than the calendar suggests, because RAMQ excludes several categories of days. The Régie publishes the counting method and a calculation tool on its page explaining how to tally your absences from Québec. Understanding these exclusions often means the difference between a compliant snowbird season and a lost year of coverage.
- Departure and return days: the day you leave and the day you come back are not tallied.
- Short absences: any absence of 21 consecutive days or less is excluded from the count entirely.
Therefore a series of two-week vacations, however frequent, never counts toward the 183 days. The rule targets long stays, not regular travel. However, a single trip of 22 days or more counts in full, minus the two travel days, so the threshold arrives faster than many people assume once trips get longer.
Exceptions to the Rule
Certain situations let you exceed 183 days in a calendar year and still keep your coverage. These exceptions must be arranged with RAMQ, not assumed after the fact. The full list appears on the Régie’s page on exceptions to the presence in Québec rule.
- Work, studies, training, or an internship: an absence for any of these purposes can exceed the threshold, and coverage may extend to an accompanying spouse and dependants if they are eligible.
- The seven-year exception: once every seven years, you may be absent 183 days or more in a calendar year for personal reasons and remain eligible.
The seven-year exception is the one snowbirds tend to rely on, and it is worth using deliberately rather than by accident. Furthermore, spending the exception on a single long winter means it is unavailable for the following six years, so plan the sequence of your seasons before you book.
What RAMQ Pays Outside Quebec
Keeping your eligibility is not the same as being covered. Even while fully eligible, you are reimbursed only in part for care received outside the province, and the ceilings are fixed in dollars rather than as a percentage of the bill. The amounts below come from the Régie’s page on services covered outside Québec.
| Type of care | RAMQ reimbursement ceiling |
|---|---|
| Hospital stay without hospitalization | $50 per day |
| Day of hospitalization | Maximum $100 per day |
| Professional services (physicians) | Quebec rates, even if you paid more |
| Hemodialysis treatment | Up to $220 per treatment |
These ceilings apply whether you are in Ontario or overseas. Consequently, the gap between what a hospital charges and what RAMQ returns is yours to absorb, and that gap grows with the length and seriousness of the episode. RAMQ states plainly that travel insurance is recommended to cover the portions it does not reimburse.
Filling the Gap With Insurance
Travel medical insurance exists precisely for the space between the ceiling and the invoice. For Quebec residents, it does two jobs at once: it covers emergency care abroad, and it covers care in other Canadian provinces, where Quebec’s position is unusual. Quebec does not take part in the interprovincial reciprocal billing agreement for physician services, so a Quebecer who sees a doctor in another province pays up front and is reimbursed only at Quebec rates.
Among the providers available to Canadians, soNomad sells travel medical coverage underwritten by National Liability & Fire Insurance Company, a Berkshire Hathaway company, through Berkshire Hathaway Travel Protection. Its plans cover emergency medical care for Canadians travelling outside their home province or abroad, and pre-existing conditions can be covered subject to eligibility and stability requirements assessed through a health questionnaire. For a broader view of what these policies include, see our guide to what travel insurance covers.
- Check the trip length: annual multi-trip plans cap each individual trip, so a full winter away needs a single-trip policy of matching duration.
- Declare pre-existing conditions: a condition that is not stable during the required period before departure is generally excluded, and that exclusion is discovered at claim time.
- Do not rely on a card alone: coverage embedded in a credit card is usually limited by age and trip length. Our guide to credit card travel insurance sets out those limits.
Bottom Line
The RAMQ rule is simple to state and easy to breach: 183 days of absence in a calendar year costs you a full year of eligibility and forces you to repay the care you received. Short trips of 21 days or less never count, travel days never count, and work or study absences have their own exception, as does a once-in-seven-years personal absence.
Staying eligible, however, is only half the job. With reimbursement capped at $100 per day of hospitalization, the plan was never designed to carry a serious medical episode abroad. Price a travel medical policy against the length of your actual trip, confirm how your pre-existing conditions are treated, and keep your own record of dates. Frequently Asked Questions about RAMQ absences
How many days can I be outside Quebec without losing RAMQ?
You must not be absent from Quebec for 183 days or more in a calendar year, from January 1 to December 31. The absences can be consecutive or spread across several trips. Reaching that total means you are no longer considered a person who resides in Quebec for that year.
Do short trips count toward the 183 days?
No. Any absence of 21 consecutive days or less is excluded from the tally entirely. The day you leave and the day you return are also excluded, on every trip. As a result, frequent short vacations do not put your coverage at risk, however many you take in a year.
What happens if I exceed 183 days of absence?
You lose your eligibility for the entire year of the absence, not only for the days spent away. In addition, you must reimburse the covered services you received during that year. Because the loss is retroactive, an absence that ends in December can undo coverage used the previous spring.
What is the seven-year exception?
Once every seven years, you may be absent from Quebec for 183 days or more in a calendar year for personal reasons while remaining eligible for health insurance. It is the exception long-stay snowbirds generally use. Since it can only be used once in that period, plan which winter it applies to.
Does RAMQ cover medical care I receive abroad?
Only in part, and up to fixed ceilings. RAMQ reimburses $50 per day for a hospital stay without hospitalization and a maximum of $100 per day of hospitalization. Professional services are reimbursed at Quebec rates even if you paid more, and hemodialysis is reimbursed up to $220 per treatment.
Am I covered in other Canadian provinces?
Not in the way residents of other provinces are. Quebec does not participate in the interprovincial reciprocal billing agreement for physician services, so you pay a doctor in another province up front and claim afterwards, reimbursed at Quebec rates. Travel insurance covers that gap within Canada as well as abroad.
Do I need to tell RAMQ before a long absence?
Yes, if you are leaving for an extended period or relying on an exception. RAMQ provides a process to inform it of a departure from Québec, and exceptions for work, studies, training, or an internship need to be arranged rather than claimed retroactively. Confirm your situation with RAMQ before you go.
